Glaucoma & Diabetic Patients and Relatives Of Glaucoma Sufferers

(Evidence of Eligibility)


Glaucoma & Diabetic related categories -

  • Suffering from Diabetes, Glaucoma or considered to be at risk of Glaucoma
  • Aged 40 or over and the parent / brother / sister / child of a Glaucoma sufferer

People with Diabetes, Glaucoma or people medically at risk from Glaucoma

If a person in this category has been for a sight test to the practice before, then no evidence is required in order to claim a free sight test. Regular sight tests for people in this category are extremely important.

The NHS advises that people visiting the practice for the first time for a test, should supply one of the following -

Glaucoma Sufferers

  • A statement from your GP or the eye clinic confirming that you suffer from Glaucoma ( Such a document can also help the Optician when testing your eyes, as it will indicate the exact nature of your Glaucoma)

Diabetic Patients Should try to provide one of the following -

  • A statement from your GP
  • Repeat Prescriptions Card
  • Out Patient Appointment Card
  • Record of blood sugar readings

Aged 40 or over and the parent / brother / sister / child of a glaucoma sufferer

For patients who are related to a Glaucoma sufferer and have come to the practice for the first time should bring with them a confirmation from their GP, or consultant, or a relevant appointment card.

At present there is no documentation available to prove that a patient is a close relative of a glaucoma sufferer.

If you fall in this group, we strongly recommend Annual Sight Test - as the health of your eyes may depend on it.
