Working Tax Credit

Disabled Persons Tax Credit

(Evidence of Eligibility)

If you are on Working Tax Credit and qualify as shown below - The NHS now requests that you supply a Tax Credit NHS Exemption Certificate or your WFTC Award Notice

If you are on Disabled Persons Tax Credit and qualify as shown below - The NHS now requests that you supply a Tax Credit NHS Exemption Certificate or your DPTC Award Notice.

If you receive Working Families Tax Credit or Disabled Persons Tax Credit your average earnings are calculated according to strict rules laid out by the Government and your letter of explanation will look something like this.

Example 1.

The Money you have coming in each week has been calculated as more than:

£91.45

The maximum your family can get each week is:

£169.90

(This will be based upon your income and the number of people in your family including children)

We have worked out how much has to be taken off the Maximum

*This has to be 55% of the difference between £130.00 and the £91.45 which you have coming in each week, which is (130 less 91.45 multiplied by 55 then divided by 100)

£21.20

*If the answer to the calculation here is reduced by less than £70 as in this example, then you are Entitled to FULL help.

*You MUST take your certificate with you when you wish to claim.

The working families Tax credit you are entitled to is 139.70

In the above example you would receive a tax credit payment with your pay each week and you would be entitled to receive full help with NHS services because the WFTC has been reduced by less than £70 per week.

Example 2.

The Money you have coming in each week has been calculated as more than.

£91.45

The maximum your family can get each week is:

£169.90

(This will be based upon your income and the number of people in your family including children)

We have worked out how much has to be taken off the Maximum

*This has to be 55% of the difference between £245.00 and the £91.45 which you have coming in each week, which is (245 less 91.45 multiplied by 55 then divided by 100)

£84.45

*If the answer to the calculation here is reduced by less than £70 as in this example, then you are Entitled to PARTIAL help.

You MUST take your certificate with you when you wish to claim.

The working families Tax credit you are entitled to is

£76.45

In the above example you would receive a tax credit payment with your pay each week, but you would be entitled to receive partial help with NHS services because the WFTC has been reduced by less than £70 per week. Your chosen NHS service provider will need to see your certificate to tell you how much help you will receive.
